You can pay for your membership by check, cash or credit card. Just follow these steps:

1. Print this page and fill out the following information:
   • Name:
   • Address:
   • City/State/Zip Code
   • Home Phone
   • Work Phone
   • Other Phone
   • Birthdate (so we can include you in our birthday celebrations!)
   • Email address
2. To pay by check: write out a checks for $240 to The Human Potential Center and mail it -- along with your printout of this page -- to The Human Potential Center, 2007 Bert Avenue, Austin, TX 78704-7527.
3. To pay by cash or credit card: bring your cash or credit card -- along with your printout of this page -- to the Center next time you visit, and we'll process it then.
4. That's all there is to it! Welcome to the Center!
