You can pay for your membership with three post-dated checks. Just follow these steps:

1. Print this page and fill out the following information:
   • Name:
   • Address:
   • City/State/Zip Code
   • Home Phone
   • Work Phone
   • Other Phone
   • Birthdate (so we can include you in our birthday celebrations!)
   • Email address
2. Write out three checks for $80 each to The Human Potential Center. Date the first one for this month, the second one for next month, and the third one for two months from now. We'll cash each check no sooner than the date it is written out for.
3. Place all three checks -- and your printout of this page -- into one envelope, and mail it to The Human Potential Center, 2007 Bert Avenue, Austin, TX 78704-7527
4. That's all there is to it! Welcome to the Center!
